Claude Code 2.1.191: /rewind past /clear, ~37% less CPU while streaming

/rewind can now resume a conversation from before /clear was run, and streaming CPU usage drops roughly 37% by coalescing text updates to 100ms. Also fixes hooks with comma-separated matchers silently never firing, and background agents resurrecting after being stopped.
