Claude Code 2.1.202: dynamic workflow size setting, /review reverts to single-pass
Adds a /config setting that guides how many agents Claude spawns in dynamic workflows (advisory, not an enforced cap) and OpenTelemetry attributes for reconstructing a workflow run. /review <pr> goes back to a fast single-pass review; the multi-agent review moves to /code-review <level> <pr#>.
