Claude Code 2.1.206: /commit-push-pr auto-allows non-origin push remotes, MCP timeout fix

/commit-push-pr now auto-allows git push to the repo's configured push remote (remote.pushDefault, or the sole remote) in addition to origin, and EnterWorktree asks for confirmation before entering a worktree outside .claude/worktrees/. Fixes a per-server request_timeout_ms being ignored in fresh sessions, which timed out long MCP tool calls at the 60s default.
