opencode 1.18.8: MCP reconnect after expired SDK sessions

Core fixes centre on MCP: servers now reconnect after SDK sessions expire (including under concurrent requests), configured OAuth callback ports are honoured in mcp debug, and deprecated sampling defaults are no longer sent to newer Gemini models. Desktop gets a batch of navigation and state-refresh fixes.
